Kabanata 1: Ang Triple Board Exam.

Year 2006...... In Rizal General Hospital.....
"Kailangan nalang natin na iapproach ang hepatic pedicle upang maidesict ang parenchyma". Doctor Jason asked and nurses and assistant doctor agreed.
Meanwhile doctor proceed suddenly the vital sign of patient increase.
"Doc, 140 over 90 ang patient critical ang kalagayan niya". Nurse said to doctor.
"May namataang internal hemorrhage ang patient pero hindi matagpuan kung saan banda". Assistant doctor said to him.
"Kung gayon pala kailangan natin mahanap ang internal bleeding. Suction (use of suction) ngayon pad (use of pad) maraming dugo ang tumatagas". While the heart rate is increase until the cardiac arrest.
"Doc, cardiac arrest". Nurse said to doctor.
"Magsagawa kayo ng resuscitation". Doctor asked to the nurse and followed.
"Nagkakaroon siya ng heart arrhythmias ihanda ang defibrillators bilis!!". Nurses followed.
"Ito na po doc". Nurse gave the defibrillators.
Doctor put the defibrillators. "Ireset niyo sa 200 joules( nurses followed) ready? Clear (clear) isa pa!!".
Doctor repeat CPR. After that use defibllator again. Until the patient is dead. The doctor continued to CPR.
"Doc, tama na ho iyan. Doc, kailangan nyo po ideclare ang time of dead. 8:45 PM po doc". Nurse asked to the doctor and the doctor cried.
He told to the family of patient and the family is cried. "Patawad po". Doctor said.
"Akala ko ba ililigtas nyo po siya? Wala po kayo nagawa doc, ang daya daya nyo po!!". One of the family said.
"Masakit mawalan ng mahal sa buhay. Pero ang mas masakit yung taong inaasahan na ililigtas mo pero wala ka nagawa. Naiinis ako sa sarili ko dahil ito ang first patient ko as a resident doctor". He said to himself.
"Pre, alam mo ba na may offer ang WHO tungkol sa pag-aaral sa ibang bansa?". He friend doctor asked to him.
"Huh?! Ano ang ibig mong sabihin?". He asked to friend doctor.
."Nagbigay ng slot sa ospital na ito at ikaw ang napili. Swerte mo mapag-aaralan mo ang mga wala rito sa pilipinas at higit sa lahat parang madali lang sayo ang surgery kapag doon ka nagtraining". Friend doctor gave him a slot paper.
"Wow!! Pero sa tingin mo ba karapat-dapat ako?". He asked to friend doctor.
"Oo naman, sa loob ng 4 years as a doctor ikaw palang ang pinaka-bata na nakapag-opera sa patient. Nga pala ano pala pipiliin mo diya?". Friend doctor asked to him.
"Gusto ko lahat". He replied.
"Huh?! Imposible, kahit magawa mo yan hindi mo kaya ang triple board exam". Friend doctor said to him.
"Kakayanin ko, kaya ko naman ang training ng sa loob ng 5 years na 3 ang specialties kong hawak ito ang mga Pediatrics, General Psychiatry at Child and Adolescent Psychiatry. kapag nagawa ko iyon at napag-aralan ko silang lahat. Parang alam ko na ang buong katawan ng tao. Goal ko na iyon. Maraming salamat". He said to friend doctor.
"Ikaw naman ang gumawa niyan at hindi ako". He replied.
"Sisikapin ako para masabi ko sa sarili ko na kaya ko iligtas ang mga pasyente ng walang alinlangan".He said to himself.
After 15 years... At the Airport of Manila.
"Hayyyy, mabuti nalang at natapos na ang medical seminar kasama ang mga mahuhusay na doctors. Hoyy, Doctor Marco ano ba ang iniisip mo?". Doctor asked to him.
"Wala pero.......Lahat naman ng sinasabi nila puro alam ko na. Hindi ko na nga kailangan ng seminar dahil mukhang mas mahusay pa ako sa mga beteranong doctor diyan". He arrogantly said.
"Woow, mayabang sabi mo eii". While commute to the bus. One of the passengers was collapse. He approach to him.
"Ano ang nanagyari sa kanya?". Doctor asked to the diver.
"Natumba nalang ho siya". Replied to him. He checked him body.
"May Atelectasis siya( in his mind) kailangan hubarin ko ang neck tail niya. Magpeperform ako ng CPR". He asked to him side. he does not know. Someone was watching him. The patient is conscious.
"Mabuti at naagapan mo!!". Doctor said to him.
"Sa ngayon hintayin ang pagdating ng ambulance". He said. Someone approached him and asked.
"Bakit alam mo yun?". He asked to him.
"Doctor ako, bakit? Sino ka ba?". He asked.
"Ahhh, wala naman. Masasabi ko naman na maganda ang ginawa mo pero may Atelectasis siya ano ang dapat mong gawin kapag nag colapse siya?". The man said to him.
"Paano mo nalaman na meron siyang Atelectasis? Doctor ka ba?". He asked to man.
"Sabihin nalang natin na parang ganun na nga pero hindi ako basta basta". He laugh while ambulance arrive.
"Sino ka ba?". He asked again.
"Mas maganda mag focus ka sa patient mo at dalhin mo siya sa pinakamagandang hospital. Maliwanag". He asked to him.
While Rizal Hospital... At the press conference of the doctors.
The director,chairman and president talking each other.
"Mukhang maganda nga yan!!, magkakaroon na rin tayong isang magaling na doctor.Ano ang plano sa kanya Mr. Director Roy?". The chairman asked.
"Total naman magretetired ang chief surgeon natin malamang baka siya ang papalit as a head doctor ng operating team. Ano sa tingin mo Mr. President Lito?". Director's replied while asked to the president.
"Simple lang nakakuha siya ng pinakamataas na marka sa Triple Board Exam. Isa na siyang icon at star ng hospital. Ngunit, usap-usapan na medyo may pagkaarogante at walang pakialam sa ibang kasamahan kaya naman nanganganib siya sa akin kaya naman paki-asikaso mo nalang ang best friend mo". President replied. Director agreed.
"Wag kang mag-alala hindi niya na uulitin ang nangyari sa kanya sa June Hospital. Magkaiba naman ang amerika at pilipinas". He said to all.
"Wag ka sana gumawa ng gulo... Jason". In his mind.

Hepatic Pidicle (Porta Hepatis.)
-The cross-bar of the "H" is the porta hepatis, or hilus of the liver, which contains the hepatic ducts and the branches of the portal vein and hepatic artery.
Parenchyma
-Is the functional parts of an organ, or of a structure such as a tumour in the body.
Internal Hemorrhage.
-Internal Bleeding. Bleeding of the organs.
heart arrhythmias.
-occur when the electrical impulses that coordinate your heartbeats don't work properly, causing your heart to beat too fast, too slow or irregularly.
Atelectasis
-Is a complete or partial collapse of the entire lung or area (lobe) of the lung.
Triple Board Exam.
-combine training in three specialties in five years: Pediatrics, General Psychiatry and Child and Adolescent Psychiatry. Residents are board eligible in all three specialties at the end of training and are able to practice any of the three specialties alone or in combination.
